- When members opt in to the Medicare Prescription Payment Plan, they no longer pay at the pharmacy when filling their Part D prescriptions. Instead, the plan will pay the upfront costs and then send a monthly bill to the member. This spreads the costs out over the year to make the costs more affordable for the member.
- Please note that the costs can vary by month, based on a Medicare calculation and as the member fills additional prescriptions; costs are not equally split across each month.

- Members can opt in to the payment plan by doing one of the following:
- Calling the Customer Service phone number on the back of their ID card
- Going online and signing up through their member website account
- Completing this form and mailing it to the address in the directions (members can also access the online opt-in location, via their member website, by scanning the QR code on the form)
